Since July of 2003 our dental coverage has been through Delta Dental.
The logo above is link a link to the Delta Dental website. At this website you can search for dentists, retrieve claims information and even download a claim form. You must register yourself with the Delta website to have online access to your information (claims, eligibility, etc.).
We are enrolled in the DeltaPreferred program. Each individual is entitled to a maximum benefit of $1500.00 when using an in-network provider or $1250.00 maximum benefit when using an out-of-network provider. Our contract year runs from July 1 of a given year to June 30 of the following year. We follow the payment schedule below:
