In July, 2004 we enrolled with Davis Vision to provide our vision coverage.

You can use their website to find a participating doctor, check your eligibility, confirm enrollment of a family member, look at frames, or even print forms. To login at the Davis website click on "Member Login" at the right-hand side of the page. For the ID or login name use your SS# and then for the password use the first 5 letters of your last name.
